What is collected
Contact details you provide when booking, and clinical notes and records created during assessment and therapy.
How your information is used
Your information is used to:
- Provide psychological therapy
- Maintain accurate clinical records
- Ensure safe and appropriate care
- Manage payments and practice administration
The lawful basis for holding this data is the provision of healthcare and legitimate interest. For health information and other special category data, I rely on the UK GDPR provisions relating to the provision of health or social care, together with the relevant safeguards under UK data protection law.
Storage and retention
Your personal information and clinical records are stored securely using appropriate technical and organisational measures. I use WriteUpp, a secure practice management platform, to manage client records, clinical notes, appointments, forms, invoices and related practice administration.
Clinical records are retained for 7 years after the end of therapy, after which they will be securely deleted or destroyed, subject to any circumstances where a longer retention period is required for legal, regulatory, professional or insurance purposes. Retention periods are reviewed periodically to ensure that personal information is not kept for longer than necessary.
Confidentiality and Information Sharing
Information shared during therapy will normally be kept confidential. There are, however, circumstances where information may need to be shared without your permission. These may include:
- Where there is a serious and immediate concern about your safety or the safety of another person
- Where there are safeguarding concerns
- Where disclosure is required by law
- Where disclosure is necessary to protect against serious harm
Where possible, this will be discussed with you first. I may also discuss aspects of your work in professional clinical supervision. Information will be anonymised wherever possible and only information necessary for supervision will be shared.
Your rights
You can request access to, correction of, or deletion of your data, and you can complain to the Information Commissioner's Office at ico.org.uk.
